To order your video today, please fill out BOTH the personal information and play selection sheets below.  Mail the sheets, along with your game films and a check (bank or certified is preferred) for $350 made out to Highlight Video for 5 DVDs (additional DVDs can be purchased for $5 each) to:

If you wish to include any pictures on your DVD, please package them with your order. We will place them at our discretion in the video - if you have a personal preference, please indicate so on your personal information sheet.

We will contact you once we receive your order and provide you with a date when you can expect to receive your DVDs. Game films will be returned in your DVD package.
